    摘要:

    肝癌的发生是一个复杂的多阶段过程,涉及遗传、表观遗传以及环境等多重因素的相互作用。环境致癌物暴露、饮食致癌物暴露和社会心理等外部因素通过诱导炎症反应、氧化应激及DNA损伤等方式重塑人体内部微环境,直接或间接促进肝癌的发生发展。本综述主要阐述外部环境如何影响内部环境,两者协同变化导致肝癌的发生发展。深入解析其作用,可为高危人群提供精准暴露干预策略,进而减少肝癌发生率或延缓肿瘤进展并改善预后。

    Abstract:

    The development of hepatocellular carcinoma is a complex, multi-stage process involving genetic, epigenetic, and environmental interactions. External factors such as exposure to environmental and dietary carcinogens, as well as psychosocial factors, can directly or indirectly contribute to the initiation and progression of hepatocellular carcinoma by inducing inflammatory responses, oxidative stress, and DNA damage, which in turn remodel the internal microenvironment of the human body. This review primarily discusses how external environmental factors influence the internal environment and how their synergistic alterations lead to the initiation and progression of hepatocellular carcinoma. An in-depth analysis of these roles could provide precise exposure-based intervention strategies for high-risk populations, reducing the incidence of hepatocellular carcinoma, delaying tumor progression, and improving prognosis.
